If you have an Affinity Health Plan card in your wallet, it works here. Moses Medical accepts Affinity Medicaid Managed Care and HMO plans at our office at 871B Westchester Ave in the Bronx. Most visits cost $0. No surprise bills.

Covered Services

Affinity members at Moses Medical can see both primary care doctors and specialists in one location. Here is what your card covers at $0 for Medicaid members:

Primary care and internal medicine for physicals, sick visits, immunizations, and chronic disease management. Cardiology for heart screenings, blood pressure checks, and cardiovascular monitoring. Dermatology for skin conditions and screenings. Behavioral health for therapy, counseling, and psychiatric medication management.

Pain management for chronic pain and musculoskeletal problems. Podiatry for diabetic foot exams and routine foot care. People with diabetes in the Bronx need regular foot checks. That is why podiatry is one of the most important services on this list. Women’s health for gynecological exams, prenatal care, and family planning.

We handle lab work and radiology on-site. Your blood gets drawn and your X-rays get taken in the same building where your doctor reads the results.

How to Use Your Affinity Card Here

Bring your Affinity Health Plan card and a photo ID. That is it. If you lost your card, call Affinity at 1-866-247-5678 for a replacement or to get your member ID over the phone.

Affinity requires you to pick a primary care provider. You can assign a Moses Medical doctor by calling Affinity or by telling our front desk staff. The switch usually takes effect within a few days. If you currently have a different PCP and need care before the change processes, come in anyway. We will help you figure out next steps.

Affinity Medicaid may require a referral from your PCP before you see a specialist. Our primary care providers write referrals during the same visit. In most cases, you can see a specialist that same day or within the week.

What Affinity Members Often Miss

Behavioral health at $0. Affinity covers behavioral health with no copay for Medicaid members. Therapy, counseling, and psychiatric care are available at our office. Most members do not realize they can see a counselor in the same office where they see their doctor.

Prenatal care from the first trimester. Affinity covers women’s health services including prenatal visits, well-woman exams, and contraceptive counseling. If you just found out you are pregnant, your first call should be to schedule prenatal care. Early visits matter.

Diabetic foot exams every visit. If you have diabetes, podiatry visits for foot exams are covered and recommended. Small cuts and blisters on your feet can turn into serious problems fast. Regular exams find them before they get worse.

Preventive screenings at $0. Annual physicals, diabetes screening, blood pressure checks, cholesterol tests, and cancer screenings are covered. These are the visits that keep you out of the emergency room.
