You are juggling two kids, one of them has a fever, and you have a Fidelis card somewhere in your bag. You need a doctor today, not next Thursday. Moses Medical at 871B Westchester Ave takes walk-ins. Bring the Fidelis card, bring the kids, and we will sort out the rest at the front desk.

Fidelis Care covers three different plan types: Medicaid Managed Care, Medicare Advantage, and marketplace Qualified Health Plans through NY State of Health. We accept all three. If you are on Fidelis Medicaid, most visits cost $0. If you are on a marketplace plan, your copays depend on the tier you chose at enrollment.

Your Fidelis Benefits at Our Office

Fidelis Medicaid members pay $0 for primary care and internal medicine visits. That covers physicals, immunizations, sick visits, and ongoing management for conditions like diabetes and high blood pressure.

Your card also covers specialists at our office. Cardiology for heart health. Dermatology for skin issues. Behavioral health for therapy, counseling, and medication management. Pain management for chronic pain. Podiatry for diabetic foot exams and general foot care. Ophthalmology for eye exams and conditions like glaucoma.

Women’s health is covered for gynecological exams, prenatal care, and family planning. Bloodwork goes through our on-site lab and imaging through our radiology department. No separate trip required. Physical therapy is available when your provider orders it.

Getting Set Up

Find your Fidelis member ID card. It shows your plan type and ID number. Bring it every time.

If you are on Fidelis Medicaid Managed Care, you need a primary care provider on file. You can assign a Moses Medical doctor by calling Fidelis at 1-888-343-3547 or logging into fideliscare.org. You can also tell our front desk and they will help you switch.

Medicaid Managed Care plans usually require a referral from your PCP before you see a specialist. Our staff handles referrals same-day, so in most cases you can see a specialist during the same visit you ask about one.

If your child gets sick and you do not have an appointment, walk in. We see walk-in patients during office hours and verify your Fidelis eligibility at the desk.

Three Things Fidelis Covers That You Probably Skip

Behavioral health at $0. Therapy, counseling, and psychiatric visits are covered for Fidelis Medicaid members with no copay. You can see a behavioral health provider at our office without a separate referral on most plans.

Annual screenings you keep putting off. Diabetes, blood pressure, cholesterol, and cancer screenings are all covered at $0 for preventive visits. These tests reveal problems when they are still manageable. Skipping them means finding out in the ER.

Medicare Advantage extras. If you carry Fidelis Medicare Advantage, your plan may include transportation to appointments, over-the-counter health product allowances, and dental and vision coverage that Original Medicare does not offer. Ask our staff what your specific plan includes.
