UnitedHealthcare is the largest insurer in the country. We accept nearly every UHC plan type at Moses Medical, 871B Westchester Ave in the Bronx. HMO, PPO, EPO, Medicare Advantage, and Medicaid Community Plan. If you have a UHC card, it almost certainly works here.

The rules vary by plan type. The details below are organized so you can find what applies to you.

What Your UHC Plan Covers Here

UHC members can receive the following at our practice:

• Primary care for physicals, preventive screenings, and ongoing health management
• Internal medicine for diabetes, high blood pressure, asthma, and thyroid disorders
• Cardiology for heart evaluations, EKGs, and cardiovascular disease management
• Dermatology for skin exams, rashes, and skin cancer screening
• Behavioral health for therapy, psychiatric evaluation, anxiety, depression, and PTSD
• Pain management for chronic back pain, arthritis, and nerve pain
• Podiatry for diabetic foot care and heel pain
• Ophthalmology for medical eye exams and diabetic retinopathy screening
• ENT for sinus problems, hearing concerns, and throat conditions
• Women’s health for gynecological exams and reproductive care
• Lab services with on-site blood draws and same-day processing
• Radiology for X-rays and diagnostic imaging
• Physical therapy for post-surgical rehab and injury recovery
• Medical weight loss with physician-supervised programs
• Wound care for diabetic ulcers and chronic wounds

For UHC members managing diabetes, that means your primary care doctor, podiatrist, ophthalmologist, lab, and wound care team are all in one building.

Verifying Your UHC Benefits

Pull out your UHC card and check the plan type printed on the front. Then follow the steps for your plan:

HMO and EPO members need a PCP on file. Pick a Moses Medical doctor through uhc.com or by calling 1-800-328-5979. Your PCP writes referrals for specialist visits.

PPO members can book specialist appointments directly. No referral needed.

Medicare Advantage members should bring both their Medicare card and UHC MA card. Confirm Moses Medical is in-network for your specific plan before your visit.

Medicaid Community Plan members should bring their NY State Medicaid card and UHC Community Plan card. Our front desk verifies eligibility at check-in.

If any service needs prior authorization, our staff handles it.

Plan-Specific Tips

For employer-sponsored HMO/PPO/EPO members: Your plan covers a $0 annual wellness visit with preventive screenings, blood panels, and immunization updates. Schedule one every year with primary care.

For Medicare Advantage members: You qualify for an Annual Wellness Visit that includes a personalized prevention plan. Many MA plans also cover medical weight loss programs, hearing exams, and dental. Check your Evidence of Coverage or call UHC. For Medicaid Community Plan members: Preventive visits, lab work, prescriptions, and specialist visits are covered with no copay or very low copays.
